The City of Hollywood offers two PPO medical plans through Cigna: Open Access Plus (OAP), and Open Access Plus In-Network (OAPIN). Coverages are nationwide.

Dental Plans*
The City of Hollywood offers three PPO dental plans through Cigna: 1.) the Comprehensive Plan, which provides a $1,000 maximum benefit per year/per person, 2.) the Premium Plan, which provides for $2,000 maximum benefit per year/per person, and 3.) the Buy-up Plan which provides for $3,000 maximum benefit per year/per person.

The link above leads to the machine-readable files that are made available in response to the federal Transparency in Coverage Rule and includes negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers. The machine-readable files are formatted to allow researchers, regulators, and application developers to more easily access and analyze data.
Vision*
The City offers three vision plans through VSP (Vision Services Provider), an online-based provider: 1.) Choice Option 1, 2.) Choice Option 2, and, 3.) Signature Option 3. VSP does not provide ID cards. Employees can log on to vsp.com to search for providers, view benefits, and print ID Cards. Your social security number is your member ID.
Health Reimbursement Arrangements (HRA) & Flexible Spending Accounts (FSA)*
The City's Health Reimbursement Arrangements (HRAs) & Flexible Spending Accounts (FSAs) are administered by the P&A Group. For more information, contact customer Services at 1-800-688-2611.
Health Reimbursement Arrangements (HRA)
The City of Hollywood contributes funds annually to eligible employees' HRA accounts. These funds are available on a "use it or lose it" basis, to be used for IRS-approved healthcare expenses that are not reimbursed. Funds are normally available on January 1 and expire on December 31 of the same calendar year. Unused amounts will not be carried forward. Fund amounts correspond to medical enrollment tier: Employee Only = $400, Employee +1 = $600, Employee + Family = $1,000.
Flexible Spending Accounts (FSA)
Eligible employees can also participate in the City's 125 Flexible Spending Plan, a way to set aside a portion of pre-taxed dollars to pay for qualifying out-of-pocket healthcare expenses or dependent care. These funds are elected during Open Enrollment or upon hire; they, too, expire on December 31 and are subject to a "use it or lose it" basis. Unused amounts will not be carried forward.
The maximum annual benefit amount that an employee may currently elect for Dependent Care FSA is $5,000. The maximum annual benefit amount an employee may currently elect for Healthcare FSA is $3,050 (i.e. for unreimbursed medical, prescription, and dental co-pays, deductibles, and co-insurance).

Voluntary Supplemental Insurance Programs*
The City of Hollywood offers voluntary supplemental insurance choices through employee-paid payroll deductions.
AFLAC
AFLAC provides supplemental insurance to help pay out-of-pocket expenses major medical insurance doesn't cover. AFLAC pays cash directly to the plan member after a covered claim has been successfully submitted and processed. The cash received can be spent how the plan member sees fit. AFLAC product enrollment options are Accident, Hospital, Critical Care, Critical Illness, and Short Term Disability (STD).

Preferred Legal Plan
PLP provides comprehensive legal protection at a low per-pay fee. Types of services include legal representation on a range of issues, estate planning, document review, and more. Group Life Insurance & Long Term Disability*
Basic Life & Accidental Death & Dismemberment
The City provides Basic Life and Accidental Death and Dismemberment (AD&D) insurance through Symetra. Additionally, supplemental (voluntary) life insurance and/or dependent life insurance for spouses and/or dependents are available for purchase by employees. At times, applicants for supplemental (voluntary) and/or dependent life will need to complete an Evidence of Insurability form and may be required to undergo a physical examination. All life insurance participants are encouraged to update their beneficiaries regularly.
Long Term Disability
The City provides Long Term Disability insurance through Symetra. LTD provides up to 60% of the member's monthly salary, up to $9,000 per month. This benefit will begin on the 91st day after being out of the office for 90 consecutive days due to an injury or illness.

Enrollment for Healthcare and Voluntary Supplemental Benefits
The City uses an online benefits platform, BenTek, for employees to enroll in and make changes to benefits elections. BenTek is also a resource for various plan documents, such as Summary of Benefits & Coverages, vendor flyers, Side-by-Side Plan Comparisons, Rates, and more! Hollywood Wellness
The City of Hollywood's award-winning wellness program, Hollywood Wellness, seeks to educate, encourage and empower employees and their families to improve or maintain their overall wellness through the promotion of the five dimensions of well-being: Physical, Emotional, Environmental, Social, and Financial. Hollywood Wellness offers challenges, workshops, webinars, FREE access to state-of-the-art Fitness Facilities, and other wellness initiatives. Incentives earned from participating in various wellness initiatives may include, gift cards, Apple Watches, lunch coolers, BBQ grills, portable sound spas, water tumblers, and more! Hollywood Wellness also partners with local community vendors to offer subsidized fitness memberships to employees, i.e. OrangeTheory Hollywood.
Life Scans*
The City of Hollywood pays for team members to complete one Life Scan per year, a value of $395 per scan. Life Scans are comprehensive wellness exams that can be completed within three hours in one location. Each Life Scan includes a comprehensive medical evaluation, ultrasound imaging, cardio-pulmonary assessments, vision and hearing test, and extensive laboratory blood profiles. The exam focuses on the prevention and early detection of illness or chronic conditions. Each exam provides recommendations for achieving and maintaining a long-term healthy lifestyle and managing risks to prevent fatalities.
