The City of Hollywood offers full-time and eligible part-time employees PPO health plan coverage through Cigna.  
Cigna offers two (2) PPO plans:  Open Access Plus (OAP), and Open Access Plus In-Network (OAPIN).

Online Medical, Dental and Vision Insurance Enrollments

Medical, Dental and Vision Insurance enrollments are now completed online only via  Insurance enrollment is done during the City's annual Open Enrollment period and may be done during the year for a qualifying event: marriage, divorce, birth or adoption of a child, loss of coverage of a spouse, registration or de-registration of a domestic partner.

Dental Plans

The City of Hollywood offers full-time employees PPO Dental Insurance. Employees have the option of enrolling into one of two (2) plans: the Comprehensive Plan, which provides a $1,000 maximum benefit per year/per person, or the Premium Plan, which provides for $2,000 maximum benefit per year/per person.

VSP (Vision Services Provider) is an online based provider and does not provide ID cards. Employees can log on to to search for providers, view benefits and print ID Cards. Your social security number is your member ID. VSP Enrollment Form

Health Reimbursement Arrangements (HRA)

The City of Hollywood contributes funds annually to eligible employees' HRA accounts.  These funds are available on a "use it or lose it" basis, to be used for IRS-approved medical expenses that are not reimbursed. Funds are normally available on January 1 and expire on December 31 of the same calendar year. Unused amounts will not be carried forward.

Flexible Spending Accounts (FSA)

Eligible employees can also participate in the City's 125 Flexible Spending Plan, a way to set aside a portion of pre-taxed dollars to pay for qualifying out-of-pocket medical expenses or dependent care. These funds are elected during Open Enrollment or upon hire; they expire December 31, and are subject to a "use it or lose it" basis.

The maximum annual benefit amount that an employee may currently elect to receive in the form of reimbursements for the Dependent Care Reimbursement Expense Plan is $5,000. The maximum annual combined total City and employee contribution amount for the Unreimbursed Medical Expense Plan is $2,650. (i.e. for unreimbursed medical, prescription, and dental co-pays, deductibles and co-insurance). 

Voluntary Supplemental Insurance Programs

The City of Hollywood offers voluntary supplemental insurance choices through employee-paid payroll deductions. Note: New employees have 30 days from their date of hire to enroll in these supplemental programs or wait until open enrollment. Benefit changes made during open enrollment will become effective January 1 of the upcoming year. Contact the representatives listed below for more information and/or to enroll:

Group Life Insurance


The City provides Life and Accidental Death and Dismemberment (AD&D) insurance to full time employees. In addition, supplemental (voluntary) life insurance for employees and/or dependent life insurance for spouse and/or dependents, are available for purchase by employees. Applications for supplemental (voluntary) and/or dependent life insurance will be accepted during Open Enrollment periods or a qualifying event. Applicants for supplemental (voluntary) and/or dependent life will need to complete an Evidence of Insurability form and may be required to undergo a physical examination. Symetra Life  Insurance Company will notify the City upon approval of application. All life insurance participants are encouraged to update their beneficiaries regularly.
