When requesting a Fire Run Report please call the Division of Fire Prevention & Life Safety at (954) 967-4404.
Fire run reports are available within 72 hours from the date of the fire. The cost per page is $1.00 cash or check
accepted, sorry no credit cards. Make any checks payable to the “City of Hollywood”. The report can be picked up
at 2741 Stirling Road, Hollywood third floor between the hours 08:00 am-4:30 pm, Monday through Friday.
Note: The complete report may not be available until the fire investigation is completed.
How do I obtain a copy of the Emergency Medical Report from when I was treated by the Paramedics?
Answer:
To order a rescue run report record, contact the Support Services Office at (954)967-4405 or to request
in writing to Hollywood Fire Rescue, 2741 Stirling Road, Hollywood, Florida 33312, attention EMS Records.
Provide patient name, date of incident, and location of incident if you know it and a telephone number or
email address where you can be contacted.

Generally, we respond to requests for access to PHI within 5 working days, unless the designated record is
not maintained at our facility. If you are not the patient then you must have a HIPPA compliant subpoena

The Fire Rescue Department provides free installation of battery operated smoke detectors. To schedule an

The Division of Fire Prevention & Life Safety at (954) 967-4404 Monday through Friday from 08:00 am - 4:30 pm.
This office is closed on all legal holidays. If you need to report a fire code violation that is a serious life
safety hazard outside the normal work hours call ‘911’
