If you do not have the free Adobe Reader, click here.   Local MBE and SBE Program Application Download Document List of Commodities Download Document List of Commodities Interactive Search Form W-9 Download Document •  On-Line Application An electronic version of the Local MBE/SBE application is available at the link provided above.  Please complete the application along with the List of Commodities and Form W-9 and mail it to the address provided below.  The following information must be submitted with the application.   Don't forget to please include the following with your application: All licenses to do business in Florida and in the City of Hollywood. Proof of payment of all required City of Hollywood business taxes and fees. Commodity Code sheet indicating goods/services your business is capable of providing to the City of Hollywood. Documentation that business is domiciled within the corporate limits of Hollywood, FL. (i.e. Occupational license, utility bill, lease/rental agreement, property tax receipt) Current certificates of insurance W-9 Form completed Copies of business certifications (i.e. MBE, SBE, DBE, WBE, etc) Documentation to demonstrate the business employs 50 or fewer, permanent full-time employees averaged over a two year period. Copies of prior three (3) years federal income tax returns, including all schedules and last three years' financial statements prepared by an independent CPA or Accountant.  If in business for less than three (3) years, a personal tax return is required.
