All volunteers interested in participating with City youth athletic leagues are required to complete a Youth Program Background Check Application.
1. Download application checklist.
2. Download and fill out the application.
3. Take it to the youth organization you wish to participate with for signature.
4. Come out and volunteer with us!!!!!

Athletic field use is on a first come, first serve basis unless a field permit is issued. Field permits have priority over open play. Organized games, tournaments and practices are not allowed during open use. Organized games, tournaments and practices consist of one of the following: team uniforms, referees, banners, coaching, etc. If interested in renting an athletic field please contact (954) 967-4239 for field availability and prices. A certificate of liability insurance naming the City of Hollywood is required for all field permits. Standard athletic facility rules apply and the City reserves the right to close field without notice.
