Where do I get a Liability Claim Form?

The Liability Claim Form can be downloaded, mailed or faxed to you. Forms are also available on a walk-in basis. The form must be returned in person, mailed or faxed to Risk Management.

Does it cost me anything to file a claim?

There is no cost associated with the filing of a claim.

Why can the claim form be faxed but not emailed to Risk Management?

We need to have the original signature of the person making the claim on file. A claim form may be faxed to expedite the filing of a claim, but the original must then be mailed.

What Information do I need to send in with my claim form?

As with all claims, you should provide any information that you believe will support your claim. (i.e., estimates, photographs and diagrams)

What happens to my claim?

After your claim is received by Risk Management, it is assigned to an Analyst. The Analyst reviews the claim and supporting documents. If the Analyst has any questions, he or she will contact you. If not, he or she proceeds with review of your claim. Your claim may be forwarded to an Adjuster with the City’s third party administrator, Unisource Administrators, for handling.

Once I file my claim, how long does it take to hear from Risk Management?

The Analyst and/or Adjuster will move quickly to resolve your claim. You may be contacted for clarification or further information; however, as soon as all the necessary information has been received, you will be advised if your claim is approved or denied.

I saw a pothole in the street that could cause an accident. Is Risk Management the place to call to report this so it can be fixed?

Yes However, if you see a condition in the City that creates a potential hazard to the public you should immediately contact Public Works at (954) 967.4526. They will be able to dispatch the appropriate City crew to investigate and resolve the problem without delay.
