Any restaurant that wishes to establish outdoor dining areas in public or private in the historic downtown on Hollywood Boulevard or on the Hollywood Beach Broadwalk must submit their Certificate of Insurance and Indemnification form for Temporary Café Zones and provide the following information.
Use of the Temporary Café Zone program requires proof of insurance. Please upload a Certificate of Insurance which lists the City of Hollywood as an additional insured. The requirements for coverage are:
• Not less than $1,000,000.00 per occurrence for bodily injury and property damage, $2,000,000 aggregate
• Coverage should include Products and Personal Liability coverage.
